2024 BENEFITS PROGRAM
Your benefits are designed to provide you and your family with affordable, easily-accessible plan options so that you have the security you need now and in the future. In 2024, Famous Supply will continue to offer health care, dental, vision and other benefits with the flexibility that meets your unique needs.
SUMMARY
ELIGIBILITY REQUIREMENTS
• Must be full-time working 30 hours or more per week.
• Must be employed at Famous for 60 days.
Benefits will be effective on the 1st of the month after your 60 days. Dependent coverage is available but some eligibility restrictions may apply. Dependents may include your spouse and children under the age of 26.
WHEN CAN I ENROLL?
• During your initial 60 days of being hired.
• During the annual open enrollment.
• Within 30 days of a qualified family status change.
If you do not enroll at the above times, you must wait for the next annual open enrollment period.
WHAT IS THE NEW HIRE ENROLLMENT PROCESS?
• Review all benefit information enclosed. If you would like more detailed plan information, login to your ADP self-service portal for plan summary documents. Instructions to login are enclosed.
• Login to ADP by your 30th day of full-time employment.
• Send a signed confirmation statement to HR by your 30th day of full-time employment.
• Review your paycheck after your benefit effective date to confirm all deductions are accurate.
MAKING CHANGES
After your new hire enrollment period ends, changes to all benefits may be made during the annual open enrollment period. Family or enrollment status changes also allow you to make a change to your benefits, including but not limited to:
• Marriage / Divorce
• Birth or adoption of an eligible dependent
• Death of a spouse or covered dependent
• Loss of other coverage (for associate, spouse or dependent)
• Gain of other coverage (for associate, spouse or dependent)
• Court-ordered coverage change
• Receiving a Qualified Medical Child Support Order (QMCSO)
If you have a family or enrollment status change, you must notify your HR representative and complete the necessary forms within 30 days of the event date.
